Is using 'Hitler', 'Adolf' or anything else like that offensive?

If yes, is it so offensive that it should be censored?

If yes, should the culprit be placed under house arrest or merely blackmailed with bogus prosecutions for tax offenses?

The most self-loathing statement is "I am offended by that". Whenever anyone uses something along similar lines, they prove themselves base, saving me the effort.

The owners of the site can ban anyone they want. If there is a name that causes a stir in the community, then the owners may ban that name; but offer the user the option to make a new account and new name.

There are more names that get censored than just Hitler and Adolf, btw.

It depends on what you mean by that. There are 3 components to any message: what the sender intends, the actual words used and what the hearer perceives.

To me the middle portion doesn't matter. If I say bitch or AoG, and they both mean the same thing, then it really doesn't matter.

Well if you were to show me a picture of two gay men having sex I would find that distasteful. Not because I have any issue with two gay men having sex, but simply because that does not suit my taste.

Similarly if you say you think a womans place is in the kitchen then I will find that to distasteful. This time I probably will take issue with misogyny, but that's because typically in misogynic relationships there is an element of exploitation and/or power asymmetry.

I find a lot of people state they are offended don't really mean they are offended, but are simply trying to communicate their dislike and ensuring that dislike is widely recognized (along with whatever they dislike being publicized).

Usernames should be censored exactly as much as any general content should be censored. If the site is geared towards children, education, or functions as a proper professional setting, they have the right to ban any content they deem as offensive.

The only stated rules of this forum are the community guidelines, so it depends. Is the name Adolf Hilter violating the rules of the forum?

Trolling? Maybe, depending on the intent.
Bigoted? Not in and of itself, once again it depends on the context.

If a name itself meant anything offensive without context, we would have to censor history books and articles. If the user is using it for satire, or playing a character for historically themed matches, I say it's acceptable under the rules. If they obviously have genuine admiration for Hitler's ideals and are promoting anything bigoted, them ban them for bigotry.

Then again, from what I've been seeing in some threads lately, is it even possible to get banned for forum related offenses anymore?

I guess I'll have to do the dishes then. And cook. But there would be some things that I will not do because I can't do them. I may have to work during those times.
